Photo Donation Acknowledgement & Agreement

In order to maintain agreement and consistency of use of a donated digital photographic image, JensenOne Creative and Mike Jensen Photography require all recipients of donated digital photographic images to complete the following form.

All images used for digital projection to be credited as follows: Photo by Mike Jensen | MikeJensen.Photography ~ Font used must be readable by the general audience of the presentation.

Watermark to be applied by Mike Jensen Photography All images used for the internet must be provided by Mike Jensen Photography and must be copyrighted and credited by Mike Jensen ~ Watermark file to be placed on image by Mike Jensen or his designees to be readable by the general audience.
